  • Hydroxychloroquine has been added to the Conditional Risk List for the conditions of hypokalemia, hypomagnesemia, bradycardia, excessive dosage and concomitant use with other QT prolonging drugs. 

  • Trimethoprim Sulfa: When the QTdrugs list was created in 2000, this drug combination was placed on the list of drugs with Conditional Risk of TdP.  This was based on two convincing reports in the medical literature that, in retrospect, could possibly have been confounded by congenital long QT or other factors.  Since no further reports or evidence of TdP have become available and the data in the FDA's AERS have not substantially changed, the drug combination has been moved from the Conditional List to the Drugs to the list of drugs to Avoid in patients with Congenital Long QT.
